Job summary

The Lakes Home Care, Inc. is seeking a Licensed Practical Nurse (LPN) to provide in-home care to pediatric and/or adult patients in Florida. You will assess, plan, implement, and evaluate nursing care following prescribed orders.

Responsibilities include monitoring patient conditions, collecting data, and documenting nursing diagnoses while delivering compassionate, patient-centered care in the home setting.

Qualifications

• Currently licensed as an LPN/LVN in the state where the LPN will practice.
• One year of professional nursing experience preferred.
• BLS (Basic Life Support) certification is desirable or required

• On each assignment, provides skilled nursing care/services in accordance with prescribed orders
• Assess signs and symptoms indicating physiologic and psychosocial changes in the patient’s condition.
• Collects, analyzes, and interprets data and information from health care members and documents actual and/or potential nursing diagnoses.

Qualifications:

• Currently licensed as an LPN/LVN in the state in which the LPN will practice.
• Current TB or Chest X-Ray.
• Current BLS card.
• One year prior professional nursing experience preferred
