Job summary

The Lakes Home Care, Inc. seeks a Licensed Practical Nurse (LPN) to work with pediatric and/or adult patients in their own homes, delivering skilled nursing care per prescribed orders in a home health setting.

Responsibilities include assessing and planning care using the nursing process, monitoring patients for physiologic and psychosocial changes, collecting and documenting data, and communicating findings with the health care team.

Qualifications

• Currently licensed as an LPN/LVN in the state of practice.
• Current TB or Chest X-Ray.
• Current BLS card.
• One year prior professional nursing experience preferred
