Job summary

TGH Rehabilitation Hospital is seeking an LPN to join our acute inpatient rehabilitation team in a PRN/varied schedule. You will deliver direct and indirect patient care, coordinate with an interdisciplinary team, and support safe discharge planning to help patients regain independence.

Applicants should have an ADN or nursing diploma and an active LPN license, with CPR/BCLS certification; ACLS is preferred.

Qualifications

• Graduation from an accredited Associate Degree in Nursing or Nursing Diploma program.
• Current state licensure as Licensed Practical Nurse (LPN).
• CPR/BCLS certification; ACLS preferred.

Responsibilities

• Deliver direct and indirect patient care using the nursing process: assessment, planning, intervention, and evaluation.
• Collaborate with patients, families, and interdisciplinary care team to create personalized care plans.
• Monitor and communicate changes in patient conditions to physicians and supervisors.
• Play a key role in discharge planning for safe transitions.

How You'll Make an Impact As a Licensed Practical Nurse,

• Deliver direct and indirect patient care using the nursing process-Assessment, Planning, Intervention, and Evaluation

Qualifications and requirements:

• Graduation from an accredited Associate Degree in Nursing or Nursing Diploma program.
• Current state licensure as Licensed Practical Nurse.

Additional requirements include:

• CPR/BCLS certification. ACLS preferred.
• Minimum six months' Medical/Surgical experience in an acute care setting preferred.
• Excellent oral and written communication and interpersonal skills.
