Job summary

Baptist Health Rehabilitation Hospital in Florida is seeking a Licensed Practical Nurse (LPN) for full-time day shifts. You will provide direct and indirect patient care, participate in planning and discharge processes, and collaborate with an interdisciplinary team to support rehabilitation goals.

Qualifications

• Current state licensure as Licensed Practical Nurse (LPN).
• CPR/BCLS certification; ACLS preferred.
• Minimum six months of Medical/Surgical experience in an acute care setting.
• Excellent oral and written communication and interpersonal skills.

Responsibilities

• Deliver direct and indirect patient care using the nursing process—Assessment, Planning, Intervention, and Evaluation.
• Collaborate with patients, families, and the interdisciplinary care team to create personalized care plans.
• Monitor and communicate changes in patient conditions to physicians, supervisors, and colleagues.
• Play a key role in the discharge planning process to support safe, successful transitions.
