Job summary

TGH Rehabilitation Hospital is seeking a Licensed Practical Nurse to join our rehabilitation team. You will provide direct and indirect patient care as part of the nursing process, collaborate with patients and families, and participate in discharge planning to ensure safe transitions.

Qualifications

• Graduation from an accredited nursing program (RN/BSN/ADN or Diploma).
• Current state licensure as Licensed Practical Nurse.

Responsibilities

• Provide direct and indirect patient care using the nursing process.
• Collaborate with patients, families and interdisciplinary team to create care plans.
• Monitor patient conditions and communicate changes to physicians and team.
• Assist with discharge planning to support safe transitions.

How You'll Make An Impact

As a Licensed Practical Nurse, you'll be a trusted partner in our patients' recovery journeys. You will:

• Deliver direct and indirect patient care using the nursing process-Assessment, Planning, Intervention, and Evaluation
• Collaborate closely with patients, families, and our interdisciplinary care team to create personalized care plans
• Monitor and communicate changes in patient conditions to physicians, supervisors, and colleagues
• Play a key role in the discharge planning process to support safe, successful transitions

Qualifications And Requirements

• Graduation from an accredited Bachelors of Science in Nursing, Associate Degree in Nursing or Nursing Diploma program.
• Current state licensure as Licensed Practical Nurse.

Additional Requirements Include

• CPR/BCLS certification. ACLS preferred.
• Minimum six months' Medical/Surgical experience in an acute care setting preferred.
• Excellent oral and written communication and interpersonal skills.
