North Florida Rehabilitation Hospital, part of Lifepoint Health, is seeking an experienced Licensed Practical Nurse (LPN) to join our rehabilitation team in Jacksonville. This FT day position runs 6:45 AM – 7:15 PM on an every-other-weekend schedule, with a supportive, patient-centered culture.

As an LPN, you will provide comprehensive care under RN supervision, assist with assessments, administer treatments, document accurately, and collaborate with a multidisciplinary team to promote rehab

Qualificações

• Graduation from an accredited licensed practical/vocational nurse program.
• Current state LPN licensure.
• Minimum of one year of acute care LPN experience.
• CPR/BCLS certification; ACLS preferred.
• CRRN preferred.
• Excellent oral and written communication and interpersonal skills.

Responsabilidades

• Assists with patient assessments and contributes to care planning
• Administers treatments and medications within scope of practice
• Supports interdisciplinary care interventions focused on rehabilitation progress
• Documents patient information accurately and in a timely manner
• Provides care appropriate to the age and condition of the patient
• Demonstrates strong clinical judgment, professionalism, and patient advocacy

How You'll Contribute

Provides patient care services that meet the psychosocial, physical and general aspects of care; meets the communication needs of patient and family; provides care that reflects initiative and responsibility indicative of professional expectations, under the supervision of a registered nurse.
