Job summary

St. Joseph's Living Center in Connecticut is seeking a Licensed Practical Nurse (LPN) to join our compassionate care team. We offer 12-hour shifts with a 7:00 AM–7:00 PM option and a $4,000 sign-on bonus for eligible applicants.

As an LPN, you will administer medications, monitor vital signs, and follow nursing care plans while assisting residents with ADLs, ensuring dignity and independence. Join us to support safe, high‑quality resident care in a collaborative environment.

Qualifications

• Active, unencumbered LPN licensure.
• Strong clinical skills in medication administration and vital signs.
• Knowledge of long-term care regulations and infection control.
• Current CPR certifications (BLS and ACLS).
• Fluent in English, both written and spoken.

Responsibilities

• Provide direct resident care including administering medications, monitoring vital signs, and following nursing care plans.
• Assist residents with ADLs while promoting dignity and independence.
• Monitor and document residents' clinical status and report significant changes.
• Collaborate with interdisciplinary care teams to implement and update care plans.
• Ensure infection control and safety protocols are followed.

$4,000 Sign-On Bonus!

$33.50 - $36.50/hour, based on experience Shift Differentials: $3.00/hour - evenings and overnights, $1.00/hour - all weekend shifts Full-Time| Part-Time

Delivering Solutions that Matter:

• Provide direct resident care including administering medications, monitoring vital signs, and performing prescribed treatments in accordance with physician orders and nursing care plans.
• Assist residents with activities of daily living (ADLs) such as bathing, dressing, grooming, feeding, mobility support, and toileting while promoting dignity and independence.
• Monitor and document residents' clinical status, including changes in condition, wound care progress, intake/output, and responses to treatments, reporting significant changes.
• Collaborate with interdisciplinary care teams to implement and update individualized care plans and ensure continuity of care.
• Ensure compliance with infection control protocols, safety regulations, and facility policies while maintaining a clean, safe environment for residents and staff.

Qualifications for Success:

• Active, unencumbered MA Licensed Practical Nurse (LPN) licensure
• Strong clinical skills in basic nursing procedures, including medication administration, vital signs monitoring, and documentation in electronic health records (EHR).
• Knowledge of long-term care regulations, infection control standards, and patient safety protocols in a skilled nursing or similar healthcare setting.
• Current CPR certifications, including BLS and ACLS.
• Ability to communicate fluently in English – both verbally and written
