Job summary

St. Joseph's Living Center in Connecticut is seeking a Licensed Practical Nurse (LPN) to provide direct resident care, administer medications, monitor vital signs, and follow nursing care plans.

This full-time/part-time/PRN role offers shift differentials, a $4,000 sign-on bonus, comprehensive benefits, and opportunities to work in a supportive long-term care environment.

Qualifications

Responsibilities

• Provide direct resident care including medications, vital signs, and prescribed treatments.
• Assist residents with ADLs (bathing, dressing, grooming, feeding, mobility, toileting).
• Monitor and document clinical status, wound care, intake/output, responses to treatments.
• Collaborate with care teams to update individualized care plans.
• Ensure infection control and safety compliance in the facility.

Licensed Practical Nurse - LPN | St. Joseph's Living Center $33.50 - $36.50/hour, based on experience Shift Differentials: $3.00/HR - evenings and overnights, $1.00/HR - all weekend shifts $4,000 Sign-On Bonus!

Delivering Solutions That Matter

• Provide direct resident care including administering medications, monitoring vital signs, and performing prescribed treatments in accordance with physician orders and nursing care plans.
• Assist residents with activities of daily living (ADLs) such as bathing, dressing, grooming, feeding, mobility support, and toileting while promoting dignity and independence.
• Monitor and document residents' clinical status, including changes in condition, wound care progress, intake/output, and responses to treatments, reporting significant changes.
• Collaborate with interdisciplinary care teams to implement and update individualized care plans and ensure continuity of care.
• Ensure compliance with infection control protocols, safety regulations, and facility policies while maintaining a clean, safe environment for residents and staff.

Qualifications For Success

• Active, unencumbered MA Licensed Practical Nurse (LPN) licensure
• Strong clinical skills in basic nursing procedures, including medication administration, vital signs monitoring, and documentation in electronic health records (EHR).
• Knowledge of long-term care regulations, infection control standards, and patient safety protocols in a skilled nursing or similar healthcare setting.
• Current CPR certifications, including BLS and ACLS.
• Ability to communicate fluently in English – both verbally and written
