Job summary

Jobtailor is seeking a Licensed Practical Nurse to provide professional nursing care in a Minnesota long-term care setting. The role supports residents of all ages under supervision, adheres to care plans, and collaborates with an interdisciplinary team to maintain safety and quality.

The ideal candidate holds an LPN license with state or multi-state privileges, has 3+ months of LTC experience, and is committed to ongoing CEUs and competency. Weekend shifts and monthly hours are required.

Qualifications

  • 3 months experience in long-term care (6 months preferred).
  • Minimum of 30 hours per month.
  • One weekend shift per month.
  • Graduate from accredited practical nursing program as LPN.
  • Minimum of 18 years of age.
  • Pre-licensure program with supervised clinical experience across life span, or 400+ hours as LPN at another facility.
  • Currently holds an unencumbered LPN license with State Nursing board and/or multi-state licensure privileges.
  • Maintains required department-specific competencies and certifications.
  • If IV therapy/management is required, ND-specific IV therapy education may apply.

Responsibilities

  • Provide professional nursing care for residents of all ages in long-term care under supervision of a RN, APN, or physician.
  • Apply nursing principles and age- and population-specific standards according to care plans.
  • Perform testing to evaluate data reflective of residents’ status and provide needed care.
  • Recognize problems, recommend solutions, and respond effectively in emergent situations.
  • Ensure residents’ health, comfort, and safety.
  • Contribute to resident assessments and administer ordered medications or treatments.
  • Provide technical support to healthcare professionals.
  • Document resident interactions and outcomes related to care, medication, behavior, activity, and dietary needs.
  • Establish and maintain effective working relationships with residents, healthcare providers, and the public.
  • Work as a member of an interdisciplinary care team.
  • Communicate clearly and maintain confidentiality of sensitive information.
  • Counsel residents, family members, and/or resident advocates on prevention and treatment plans.
  • Develop resident self-care programs using resident education principles.
  • Perform other related duties as assigned.

Requirements
