Job summary

Jobtailor is seeking a licensed practical nurse (LPN) for a long-term care facility in Minnesota. You will provide nursing care under supervision, perform resident assessments, administer medications, and support an interdisciplinary team to ensure resident health, safety, and comfort.

The role requires LTAC experience, ability to work weekend shifts, and ongoing CEU/compliance. A valid LPN license and eligibility for multi-state licensure are essential, with IV therapy skills preferred for

Qualifications

  • 3 months experience in Long Term Care required (6 months preferred).
  • Minimum of 30 hours per month.
  • One weekend shift per month.
  • Graduate from an accredited practical nursing program as a Licensed Practical Nurse.
  • Minimum of 18 years of age.
  • Pre-licensure nursing program including supervised clinical experience across the life span, or minimum of 400 hours of experience as LPN at another facility.
  • Currently holds an unencumbered Licensed Practical Nurse (LPN) license with State Nursing board and/or possess multi-state licensure privileges.
  • Maintains required department specific competencies and certifications
  • If position requires IV therapy/management skills in North Dakota: graduation from a Board approved AASPN program including IV therapy or completion of an approved IV Therapy course

Responsibilities

  • Provides professional nursing care for residents of all ages in long term care under the supervision of a registered nurse, advanced practice provider, or physician
  • Demonstrates nursing knowledge and competency within the full scope of practice
  • Performs testing to evaluate resident status and provides needed care
  • Recognizes problems, recommends solutions, and responds effectively in emergent situations
  • Ensures resident health, comfort, and safety
  • Contributes to resident assessments and administers ordered medications or treatments
  • Documents resident interactions and outcomes related to care, medication, behavior, activity, and dietary needs
  • Establishes effective working relationships with residents, healthcare providers, and the public
  • Works as a member of an interdisciplinary care team
  • Communicates clearly and maintains confidentiality
  • Counsels residents, family members, and/or resident advocates under clinical supervision on prevention and treatment plans
  • Develops resident self-care programs based on education principles
  • Performs other related duties as assigned
