Job summary

Jobtailor is seeking a Licensed Practical Nurse (LPN) to provide professional nursing care in long-term care facilities under supervision. You will perform assessments, administer medications, and collaborate with an interdisciplinary team to ensure resident health and safety.

The position requires 3 months of LTC experience (6 months preferred), a Minnesota LPN license, and willingness to travel within a 75-mile radius, with at least 30 hours per month and one weekend shift per month.

Job description

• Provide professional nursing care for residents of all ages in long-term care under the supervision of a registered nurse, advanced practice provider, or physician
• Demonstrate nursing principles, competency, and age- and population-specific standards according to care plans
• Perform testing and evaluate data reflecting residents' status
• Recognize problems, recommend solutions, and respond effectively in emergent situations
• Ensure residents' health, comfort, and safety
• Contribute to resident assessments and administer ordered medications or treatments
• Provide technical support to healthcare professionals
• Document resident interactions and outcomes related to care, medication, behavior, activity, and dietary needs
• Establish and maintain effective relationships with residents, healthcare providers, and the public
• Work as a member of an interdisciplinary care team
• Communicate clearly and maintain confidentiality of sensitive information
• Counsel residents, family members, and/or resident advocates under clinical supervision on prevention and treatment plans
• Develop resident self-care programs using resident education principles
• Travel to multiple locations within a 75-mile radius of the permanent address
• Perform other related duties as assigned

Requirements

• 3 months experience in Long Term Care required (6 months preferred)
• Travel required to support multiple locations within 75-mile radius of permanent address
• Minimum of 30 hours per month
• One weekend shift per month
• Graduate from an accredited practical nursing program as a Licensed Practical Nurse
• Minimum of 18 years of age
• Pre-licensure nursing program including supervised clinical experience across the life span, or minimum of 400 hours of experience as LPN at another facility
• Currently holds an unencumbered Licensed Practical Nurse (LPN) license with State Nursing board and/or possess multi-state licensure privileges
• Required department-specific competencies and certifications
• Required annual continuing education units (CEU) and competency training requirements
• If position requires IV therapy/management skills in North Dakota: graduation from a Board-approved AASPN program with IV therapy in the curriculum or completion of an IV Therapy course approved by the North Dakota Board of Nursing
