Jobtailor in Minnesota is seeking a Licensed Practical Nurse (LPN) to provide professional nursing care for residents in long-term care facilities. Job description

  • Provide professional nursing care for residents of all ages in long-term care under the supervision of a registered nurse, advanced practice provider, or physician
  • Apply nursing principles and age- and population-specific standards according to care plans
  • Perform testing, evaluate resident status, provide needed care, recognize problems, recommend solutions, and respond to emergent situations
  • Ensure resident health, comfort, and safety
  • Contribute to resident assessments and administer ordered medications or treatments
  • Provide technical support to healthcare professionals
  • Document resident interactions and outcomes related to care, medication, behavior, activity, and dietary needs
  • Establish and maintain effective working relationships with residents, healthcare providers, and the public
  • Work as part of an interdisciplinary care team
  • Maintain confidentiality of sensitive information
  • Counsel residents, family members, and/or resident advocates under clinical supervision on prevention and treatment plans
  • Develop resident self-care programs based on education principles
  • Perform other related duties as assigned
Requirements
  • Travel required to support multiple locations within 75-mile radius of permanent address
  • 3 months experience in Long Term Care required (6 months preferred)
  • Minimum of 30 hours per month
  • One weekend shift per month
  • Graduate from an accredited practical nursing program as a Licensed Practical Nurse
  • Minimum of 18 years of age
  • Pre-licensure nursing program including supervised clinical experience across the life span, or minimum of 400 hours of experience as LPN at another facility
  • Currently holds an unencumbered Licensed Practical Nurse (LPN) license with State Nursing board and/or possess multi-state licensure privileges
  • Maintains required department-specific competencies and certifications
  • Completes required annual continuing education units (CEU) and competency training requirements
  • If position requires IV therapy/management skills in North Dakota: graduation from a Board-approved AASPN program where IV therapy is part of the curriculum or completion of an IV Therapy course approved by the North Dakota Board of Nursing
