Job summary

Cortlandt Healthcare in Cortlandt Manor, NY is seeking compassionate LPNs to join our clinical team and provide high-quality care to our residents.

You will administer medications, monitor conditions, document care, and collaborate with CNAs, RNs, and physicians in a supportive, resident-focused environment. We offer competitive pay up to $40/hour and a $3,000 sign-on bonus with opportunities for professional growth.

Qualifications

• Must hold New York State LPN license.
• Previous nursing experience preferred; skilled nursing is a plus.
• Strong clinical, communication, and organizational skills.

Responsibilities

• Provide high-quality nursing care to residents per care plans.
• Administer medications and treatments as ordered.
• Monitor residents' conditions and report changes to the RN and medical team.
• Complete accurate clinical documentation.
• Collaborate with CNAs, RNs, physicians and other team members.
• Assist with assessments and care planning.
• Promote safety, dignity, and quality of life for residents.
• Communicate effectively with residents and families.
