Job summary

Cortlandt Healthcare is seeking compassionate LPNs to join our clinical team in Cortlandt Manor, NY. We offer full-time and part-time opportunities with competitive pay and a $3,000 sign-on bonus.

RNs and CNAs work closely with physicians and support staff to deliver high-quality nursing care, monitor conditions, document appropriately, and ensure resident safety and comfort.

Qualifications

• Licensed practical nurse credential required (LPN).
• Experience in patient care and nursing documentation preferred.

Responsibilities

• Provide high-quality nursing care to residents per individualized care plans.
• Administer medications and treatments as ordered.
• Monitor residents and report changes to RN/medical team.
• Complete accurate clinical documentation.
• Collaborate with CNAs, RNs, physicians, and other staff.
• Assist with assessments and care planning.
• Promote resident safety, dignity, comfort, and quality of life.
• Communicate effectively with residents and families.

What You’ll Do

• Provide high-quality nursing care to residents in accordance with individualized care plans.

• Complete accurate and timely clinical documentation.

• Work closely with CNAs, RNs, physicians, and other members of the interdisciplinary team.

• Assist with assessments, care planning, and resident needs.

• Communicate effectively with residents and their families.

What We’re Looking For

• Current New York State LPN license.

• Previous nursing experience preferred; skilled nursing experience is a plus.

• Strong clinical, communication, and organizational skills.

• Compassionate and resident-focused approach.

• Ability to work effectively as part of a team.
