Job summary

As an LPN, you will administer medications, monitor residents, document care, and collaborate with RNs, CNAs, and physicians. NY licensure required; prior skilled nursing experience a plus; strong communication and teamwork are essential.

Qualifications

• Current New York State LPN license required.
• Previous nursing experience preferred; skilled nursing experience is a plus.
• Strong clinical, communication, and organizational skills.

Responsibilities

• Provide high-quality nursing care to residents in accordance with individualized care plans.

• Complete accurate and timely clinical documentation.
• Work closely with CNAs, RNs, physicians, and other members of the interdisciplinary team.

• Communicate effectively with residents and their families.
