Job summary

St. Joseph Hospice in Thibodaux, LA is recruiting a Full-Time Licensed Practical Nurse (LPN) to join our patient‑focused hospice team.

You will provide nursing and supportive care under the guidance of the RN and coordinate care with physicians and the multi-disciplinary team. Qualifications

• Current Louisiana LPN license
• Minimum 2–3 years as LPN

• Provides nursing and supportive care under the Interdisciplinary Plan of Care, under RN direction
• Communicates with attending physicians and hospice team to coordinate care
• Document all services and submit on appropriate forms
• Demonstrates knowledge of Medicare/Medicaid hospice regulations
• Reports changes in patient condition to RN
• Exhibits great time management skills

Qualifications:

Job duties:
