Job summary

LiveWell Partners is seeking compassionate Licensed Practical Nurses (LPNs) to deliver skilled nursing and education in patients' homes, promoting independence and comfort. You will join a team of supportive clinicians and be accountable for high-quality care aligned with each patient’s plan.

Qualifications

• 1 year experience as an LPN in a home care, med surg, or ED setting.
• Current state license as a Licensed Practical Nurse.
• Proof of current CPR certification and valid Driver’s license.
• Ability to work and a flexible schedule.
• Excellent interpersonal skills.
• Active car insurance coverage.

Responsibilities

• Participate in the planning and coordination of total patient care in conjunction with the RN and the physician's plan of care, and follow the nursing care plan for each patient.
• Accurately report and record the patient's condition and care, including signs and symptoms which may be indicative of change.
• Prepare clinical/progress notes and summaries.
• Assist the patient with the activities of daily living while teaching appropriate self-care techniques.
• Follow universal precautions and all emergency procedures whenever giving any aspect of patient care.

Licensed Practical Nurse (LPN)

Compensation:Pay per visit, $40 - $45 per routine visit, Payroll is processed bi-weekly.

Shift Type:Full-time, Monday - Friday,8/9 AM - 4/5 PM, normal business hours.

Summary
