Job summary

A Licensed Practical Nurse (LPN) is needed to work one-on-one with patients in their homes, providing education and skilled nursing to promote independence. You will join a team of clinicians and maintain disciplined care to ensure high-quality outcomes.

The position is field-based, with a typical Monday–Friday schedule from 8 AM to 4 PM, including on-the-road time. Salary includes per-visit pay, mileage reimbursement, and incentives, with comprehensive benefits.

Qualifications

• One year of LPN experience in home care, med surg, or ED.
• Current state LPN license.
• CPR certification and valid driver’s license.

Responsibilities

• Coordinate with RN and physician plan of care to implement the nursing care plan.
• Accurately document patient condition and care, noting changes.
• Prepare progress notes and summaries.
• Assist with activities of daily living and teach self-care techniques.
• Follow universal precautions and emergency procedures when delivering care.

Compensation:

Pay per visit ($38 - $40 per routine visit), $2,280 - $2,400+ bi-weekly, Per visit incentive pay in addition to the minimum of the 30 visit expectation per week, Mileage reimbursed at $0.67/mile, Payroll processed bi-weekly (26 pay periods).

Shift Type:

Full-time, Monday - Friday, 8 AM to 4 PM (flexible), In the field/On the road at 15-30% of the average 40 hour work week.

Summary:

We are looking for compassionate Licensed Practical Nurses (LPNs) to work one-on-one with our patients in their home environment to give proper education and skilled nursing to allow our patients to become more independent. The LPN for this position enjoys working with a team of supportive clinicians as well as being disciplined to ensure the highest level of care.

Responsibilities:

• Participate in the planning and coordination of total patient care in conjunction with the RN and the physician's plan of care, and follow the nursing care plan for each patient.
• Accurately report and record the patient's condition and care, including signs and symptoms which may be indicative of change.
• Prepare clinical/progress notes and summaries.
• Assist the patient with the activities of daily living while teaching appropriate self-care techniques.
• Follow universal precautions and all emergency procedures whenever giving any aspect of patient care.

Qualifications:

• 1 year experience as an LPN in a home care, med surg, or ED setting.
• Current state license as an LPN.
• Proof of current CPR certification and valid Driver’s license.
• Ability to work and flexible schedule.
• Excellent interpersonal skills.
