Job summary

LiveWell Partners is seeking compassionate Licensed Practical Nurses (LPNs) to provide one-on-one nursing care in patients' homes, focusing on education and skills to promote independence. The role emphasizes teamwork with clinicians and disciplined practice to ensure high-quality care.

The LPN will work with a supportive team, participate in care planning, and adhere to safety protocols while delivering excellent patient education and hands-on care.

Qualifications

• 1 year experience as an LPN in home care, med surg, or ED setting.
• Current state license as a Licensed Practical Nurse.
• Proof of current CPR certification.
• Valid Driver’s license.
• Ability to work a flexible schedule.
• Excellent interpersonal skills.
• Active car insurance coverage.

Responsibilities

• Participate in planning and coordination of total patient care with the RN and physician’s plan of care, following the nursing care plan for each patient.
• Accurately report and record patient condition and care, including signs/symptoms indicating change.
• Prepare clinical/progress notes and summaries.
• Assist patients with activities of daily living while teaching self-care techniques.
• Follow universal precautions and emergency procedures during patient care.

Compensation: Pay per visit, Payroll is processed bi-weekly.

Shift Type: PRN, Minimum of 1-2 days per week or 6 visits across a week.

Summary

We are looking for compassionate Licensed Practical Nurses (LPNs) to work one-on-one with our patients in their home environment to give proper education and skilled nursing to allow our patients to become more independent. The LPN for this position enjoys working with a team of supportive clinicians as well as being disciplined to ensure the highest level of care.

Responsibilities
