Charwell House in Norwood, MA is seeking a compassionate LPN to join our care team. You will conduct assessments, admit and discharge residents, perform wound care, document findings, and lead CNAs and CMTs while ensuring adherence to facility protocols.

Conduct assessments, follow protocol, report medical or behavioral changes, and document findings
Perform admission duties: complete assessment, MAR, TAR, POS, review chart set-up, physician orders, and order medications
Manage discharges: complete documentation, notify physician and guardian, and arrange transportation
Provide wound care: follow protocols, irrigate, pack, and remove dressings
Lead CNAs and CMTs, ensuring compliance with facility protocols and policies
Respond effectively to emergency situations
