Job summary

Reliant Care Management Company seeks a dedicated LPN to provide compassionate resident care in Arlington, MA. You will perform assessments, follow protocols, and document changes to patient conditions.

You will supervise CNAs and CMTs, coordinate admissions and discharges, and respond to emergencies while upholding safety and quality standards. A valid LPN license is required and you must be a team player focused on patient care.

• Conduct assessments, follow protocol, report medical or behavioral changes, and document findings
• Perform admission duties: complete assessment, MAR, TAR, POS, review chart setup, physician orders, and order medications
• Manage discharges: complete documentation, notify physician and guardian, and arrange transportation
• Provide wound care: follow protocols, irrigate, pack, and remove dressings
• Lead CNAs and CMTs, ensuring compliance with facility protocols and policies
• Respond effectively to emergency situations
