Job summary

Guardian Center in Brockton, MA is seeking a compassionate LPN to support residents with their care needs and collaborate with the team.

You will conduct assessments, assist with admissions and discharges, provide wound care, lead CNAs, and respond to emergencies while following facility protocols. A valid LPN license is required.

Qualifications

• Perform admission duties: complete assessment, MAR, TAR, POS, review chart set-up, orders, and medications.
• Manage discharges: complete documentation, notify physician and guardian, arrange transportation.
• Provide wound care: follow protocols, irrigate, pack, and remove dressings.
• Lead CNAs and CMTs, ensuring compliance with facility protocols and policies.
• Respond effectively to emergency situations.
