Job summary

Heritage Place Care & Rehabilitation is seeking LPNs for full-time and part-time shifts at our 132-bed skilled nursing facility in Winchester, Tennessee. Join a compassionate team dedicated to high-quality resident care and ongoing professional development.

Qualifications

• Active LPN license in practicing state.
• Current BLS/CPR certification or willingness to obtain one.
• Nursing management experience preferred but not required.

Responsibilities

• Follows established procedure for charting and incident reporting.
• Follows company procedures for narcotic counting, pharmacy orders, and restraint reduction.
• Documents any change in patient\'s conditions including nutritional issues, infections, skin conditions, weight loss, or other changes.
• Utilizes the Quality Life Manual for patient identification, placement, and monitoring in programs.
• Document and notify applicable departments of patient room changes, LOA, new admits, or discharges.
• Makes rounds with physicians when necessary.
