Job summary

Lincolnwood Rehabilitation and Healthcare Center is seeking an LPN to provide compassionate, resident-centered nursing care in a 200-bed sub-acute and long-term care setting. You will work 7am-3pm, 3pm-11pm, or 11pm-7am; daily pay and a $3,000 sign-on bonus are offered.

Qualifications

• A current, unencumbered LPN license to practice in Rhode Island is required.
• Valid BLS (CPR) certification is required.
• Ability to manage care for multiple residents in a fast-paced setting.

Responsibilities

• Provide resident care per standards and plan of treatment.
• Follow care plans developed by Nurse Manager.
• Administer medications and treatments per plan of care.
• Monitor residents’ conditions and report changes promptly.
• Maintain accurate documentation per orders and policies.
• Collaborate with interdisciplinary teams for coordinated care.
• Ensure infection control and safety regulatory compliance.

Responsibilities for Registered Nurse (RN):

• Provide resident care in accordance with acceptable standards of nursing practice and the physician’s plan of treatment, as well as the written plan of care.
• Follow and implement care plans developed by the Nurse Manager.
• Perform medication administration and treatments in accordance with the plan of care (POC).
• Monitor and assess residents’ conditions, promptly reporting changes to appropriate clinical team members.
• Maintain accurate, timely, and complete documentation in accordance with physician orders, facility policies, and regulatory standards.
• Collaborate with interdisciplinary teams to ensure coordinated and high-quality resident care.
• Ensure compliance with infection control protocols, safety standards, and nursing regulations.

Qualifications for Registered Nurse (RN):

• A current, unencumbered active license to practice as an RN in the state of Rhode Island.
• Valid BLS Certification (CPR) required.
• Ability to manage care for multiple residents simultaneously in a fast-paced healthcare environment.
• Strong clinical assessment, communication, and documentation skills.
