Job summary

Lincolnwood Rehabilitation and Healthcare Center is actively seeking an RN for both full-time and part-time roles across multiple shifts (7am-3pm, 3pm-11pm, 11pm-7am). The facility is a 200-bed Sub-Acute and Long-Term Care center that focuses on compassionate, high-quality nursing care for resident well-being.

You will deliver skilled clinical care, monitor residents, administer medications, and collaborate with an interdisciplinary team to ensure safety and excellence.

Qualifications

• A current, unencumbered active license to practice as an RN in Rhode Island.
• Valid BLS Certification (CPR) required.
• Ability to manage care for multiple residents in a fast-paced healthcare environment.
• Strong clinical assessment, communication, and documentation skills.

Responsibilities

• Provide resident care in accordance with nursing practice standards and physician plan of treatment and written care plan.
• Follow and implement care plans developed by the Nurse Manager.
• Perform medication administration and treatments per plan of care.
• Monitor and assess residents’ conditions and promptly report changes to the clinical team.
• Maintain accurate, timely documentation per physician orders, policies, and standards.
• Collaborate with interdisciplinary teams to ensure coordinated care.
• Ensure infection control, safety standards, and nursing regulations.

Responsibilities for Registered Nurse (RN):

• Provide resident care in accordance with acceptable standards of nursing practice and the physician’s plan of treatment, as well as the written plan of care.
• Follow and implement care plans developed by the Nurse Manager.

• Monitor and assess residents’ conditions, promptly reporting changes to appropriate clinical team members.
• Maintain accurate, timely, and complete documentation in accordance with physician orders, facility policies, and regulatory standards.
• Collaborate with interdisciplinary teams to ensure coordinated and high-quality resident care.
• Ensure compliance with infection control protocols, safety standards, and nursing regulations.

Qualifications for Registered Nurse (RN):

• A current, unencumbered active license to practice as an RN in the state of Rhode Island.
• Valid BLS Certification (CPR) required.
• Ability to manage care for multiple residents simultaneously in a fast-paced healthcare environment.
• Strong clinical assessment, communication, and documentation skills.
