Job summary

Riverton Rehabilitation and Healthcare Center in Allentown, PA seeks a compassionate Licensed Practical Nurse (LPN) to join our 111-bed sub-acute and long-term care team. You will deliver safe, person-centered care, implement care plans, and support residents’ physical and emotional needs in a collaborative setting.

We offer daily pay and a competitive hourly range of $30–$36, plus benefits including health, vision, dental, 401(k) with match, and tuition reimbursement.

Qualifications

• Current, unencumbered LPN license in PA.
• Valid BLS Certification (CPR) required.
• Ability to care for and manage multiple patients in a fast-paced setting.
• Strong clinical judgment and communication skills.

Responsibilities

• Provide patient care per standards, plan of care, and physician orders.
• Follow and implement individualized care plans by Nurse Manager and team.
• Perform medication administration and treatments per plan and policies.
• Monitor residents, report changes promptly, respond to clinical needs.
• Maintain accurate documentation per orders and regulatory standards.
• Support a safe, respectful, and therapeutic environment for residents and families.

Responsibilities for Licensed Practical Nurse (LPN):

• Provide patient care in accordance with accepted standards of nursing practice, the written plan of care, and the physician’s plan of treatment.
• Follow and implement individualized care plans developed by the Nurse Manager and interdisciplinary team.
• Perform medication administration and prescribed treatments in accordance with the plan of care (POC) and facility policies.
• Monitor residents’ conditions, report changes promptly to the supervising nurse or physician, and respond appropriately to clinical needs.
• Maintain accurate, timely documentation in accordance with physician orders, care plans, and regulatory standards.
• Support a safe, respectful, and therapeutic environment for residents and their families.

Qualifications for Licensed Practical Nurse (LPN):

• Current, unencumbered active LPN license in the state of PA.
• Valid BLS Certification (CPR) required.
• Ability to care for and manage multiple patients in a fast-paced healthcare environment.
• Strong clinical judgment, communication skills, and attention to detail.
