Job summary

Logan Square Rehabilitation and Healthcare Center in Philadelphia seeks a dedicated LPN for full-time night shifts from 7 pm to 7 am. You will provide compassionate nursing care, administer medications, and document care plans for residents in a 109-bed Sub-Acute and Long-Term Care setting.

You will collaborate with the Nurse Manager and interdisciplinary team to ensure safe, person-centered care and adherence to clinical standards.

Qualifications

• Current, unencumbered active LPN license in Pennsylvania.
• Valid BLS Certification (CPR) required.
• Ability to care for and manage multiple patients in a fast-paced healthcare environment.

Responsibilities

• Provide patient care in accordance with standards and the plan of care.
• Follow and implement individualized care plans by Nurse Manager and team.
• Perform medication administration and prescribed treatments per the plan of care.
• Monitor residents' conditions and report changes promptly to the supervising nurse or physician.
• Maintain accurate, timely documentation to physician orders and care plans.

Responsibilities for Licensed Practical Nurse (LPN):

• Provide patient care in accordance with accepted standards of nursing practice, the written plan of care, and the physician's plan of treatment.

• Support a safe, respectful, and therapeutic environment for residents and their families.

Qualifications for Licensed Practical Nurse (LPN):
