Job summary

Elmhurst Rehabilitation and Healthcare Center is seeking an LPN to join a compassionate nursing team in Rhode Island. You will provide patient care aligned with the care plan and medical orders, including medication administration and ongoing assessments.

We offer full-time, part-time, and PRN opportunities with daily pay and competitive shift differentials. A Rhode Island LPN license and BLS/CPR certification are required to join our dedicated team.

Qualifications

• Active Rhode Island LPN license.
• Valid BLS CPR certification required.
• Ability to care for and manage multiple patients in a fast-paced healthcare environment.
• Strong clinical judgment and communication skills.

Responsibilities

• Provide patient care in accordance with the plan of care and physician orders.
• Follow individualized care plans developed by the Nurse Manager and interdisciplinary team.
• Perform medication administration and treatments per care plan and facility policies.
• Monitor residents' conditions, report changes promptly, and respond to clinical needs.
• Maintain accurate documentation in line with orders and regulatory standards.
• Support a safe, respectful, and therapeutic environment for residents and families.

Responsibilities for Licensed Practical Nurse (LPN):

• Provide patient care in accordance with accepted standards of nursing practice, the written plan of care, and the physician’s plan of treatment.
• Follow and implement individualized care plans developed by the Nurse Manager and interdisciplinary team.
• Perform medication administration and prescribed treatments in accordance with the plan of care (POC) and facility policies.
• Monitor residents’ conditions, report changes promptly to the supervising nurse or physician, and respond appropriately to clinical needs.
• Maintain accurate, timely documentation in accordance with physician orders, care plans, and regulatory standards.
• Support a safe, respectful, and therapeutic environment for residents and their families.

Qualifications for Licensed Practical Nurse (LPN):

• Current, unencumbered active LPN license in the state of Rhode Island.
• Valid BLS Certification (CPR) required.
• Ability to care for and manage multiple patients in a fast-paced healthcare environment.
• Strong clinical judgment, communication skills, and attention to detail.
