Job summary

STAT Home Health in Monroe, LA is seeking a Full-Time Licensed Practical Nurse (LPN) to provide in-home nursing and supportive care under the direction of the registered nurse. The role covers patient visits in the home or facility, ensuring adherence to the care plan and coordination with physicians and team members.

Candidates must hold a Louisiana LPN license, have CPR certification, at least 1 year of LPN experience, and reliable transportation.

Qualifications

• Current Louisiana LPN license is required.
• Minimum 1 year of LPN clinical experience.
• CPR certification is required.
• Must have reliable transportation for home visits.

Responsibilities

• Provide nursing and supportive care per the Interdisciplinary Plan of Care under the RN guidance.
• Coordinate with attending physicians and home health team to ensure optimal care.
• Document all services provided.
• Demonstrate knowledge of Medicare/Medicaid home health regulations.
• Report patient condition changes to the RN.

Job Duties:

• Provide nursing and supportive care in accordance with the Interdisciplinary Plan of Care, under the direction of the RN
• Communicate with the attending physicians, other home health team members, and other agencies as needed to coordinate optimal care and use of community resources for the patient/family.
• Document all services provided.
• Demonstrate knowledge of Medicare/Medicaid regulations for home health benefits.
• Report changes in patient condition and needs to RN

Qualifications:

• Current Louisiana Licensed Practical Nurse (LPN) license required
• Minimum 1 year clinical experience as an LPN
• Current CPR Certification
• Compassionate and great time management skills
• Reliable transportation to conduct patient visits at their place of residence or facility
