Responsibilities

• Provides nursing and supportive care per the Interdisciplinary Plan of Care under RN direction.
• Communicates with physicians and home health team to coordinate care and community resources.
• Documents all services provided.
• Demonstrates knowledge of Medicare/Medicaid regulations for home health benefits.
• Reports changes in patient condition to RN.
• Exhibits strong time management skills.
