Job summary

STAT Home Health in Houma, LA is hiring a Full-Time Licensed Practical Nurse (LPN) to join our patient-focused team. You will provide nursing and supportive care under the direction of the RN, and coordinate with physicians and home health staff to deliver high-quality care in the patient's home.

Qualifications

• Current Louisiana LPN license is required.
• Minimum 1 year clinical experience as an LPN.
• Current CPR Certification.

Responsibilities

• Provide nursing and supportive care in accordance with the Interdisciplinary Plan of Care, under the direction of the RN.
• Communicate with the attending physicians, other home health team members, and other agencies as needed to coordinate optimal care and use of community resources for the patient/family.
• Document all services provided.
• Demonstrate knowledge of Medicare/Medicaid regulations for home health benefits.
• Report changes in patient condition and needs to RN
