Job summary

STAT Home Health in Monroe, LA is seeking a Full Time Licensed Practical Nurse (LPN) to deliver skilled nursing care in patients' homes under RN supervision. The role includes coordinating with physicians and the home health team to ensure optimal care outcomes.

You will document services, monitor patient conditions, and respond to changes in needs while adhering to Medicare/Medicaid home health regulations and company policies.

Qualifications

• Current Louisiana LPN license required.
• Minimum 1 year clinical experience as an LPN.
• Current CPR Certification.

Responsibilities

• Provide nursing and supportive care in accordance with the Interdisciplinary Plan of Care under the direction of the RN.
• Communicate with the attending physicians, other home health team members, and other agencies as needed to coordinate optimal care.
• Document all services provided.
• Report changes in patient condition and needs to RN.
