The Elms in Milford, NH is seeking a compassionate LPN to join our team. You will provide direct nursing care, supervise nursing assistants, and coordinate day-to-day nursing activities. This role supports residents with medications, status monitoring, and safe, responsive care.

We offer part-time shifts (7AM–3PM and 11PM–7AM), competitive pay ($36.00–$42.00/hour) with shift differentials, and a comprehensive benefits package for eligible employees. Join us to make a difference close to home.

Formación

• Graduate of an accredited LPN/LVN program.
• Current unrestricted LPN license in the practicing state.
• Current CPR certification.
• Strong clinical judgment, communication, and leadership skills.
• Ability to organize priorities and remain composed in a changing healthcare environment.
• Prior long-term care experience preferred.

Responsabilidades

• Provide direct nursing care and supervise assigned nursing assistants and support staff.
• Monitor resident status and promptly communicate significant changes.
• Administer medications and monitor residents for adverse reactions.
• Perform wound care, treatments, and nursing procedures within scope of practice.
• Accurately carry out and document physician orders.
• Participate in admissions, discharges, transfers, rounds, and care planning.
• Coordinate follow-up appointments and transportation needs.
• Collaborate with residents, families, providers, and the interdisciplinary team.
• Support end-of-life care when needed.
• Promote teamwork, regulatory compliance, and survey readiness.

Compensation

$36.00 - $42.00, based on experience + applicable shift differentials

Sign On Bonus

About the Role

We're looking for an LPN who combines strong clinical skills with compassion, communication, and teamwork. You'll provide direct nursing care while helping coordinate the day-to-day work of nursing assistants and other assigned team members.
