Job summary

603 Healthcare in New Hampshire is seeking an LPN for a part-time evening shift (3PM–11PM), every other weekend, to provide direct nursing care and coordinate with CNAs and other staff in Conway, NH.

Responsibilities include monitoring residents, administering medications, documenting physician orders, and participating in admissions and care planning while collaborating with families and providers to ensure safe, compassionate care.

Qualifications

• Graduate of an accredited LPN/LVN program.
• Current unrestricted LPN license in the practicing state.
• Current CPR certification.
• Strong clinical judgment, communication, and leadership skills.
• Ability to organize priorities and remain composed in a changing healthcare environment.
• Prior long‑term care experience preferred.

• Monitor resident status and promptly communicate significant changes.
• Administer medications and monitor residents for adverse reactions.
• Perform wound care, treatments, and nursing procedures within scope of practice.

• Participate in admissions, discharges, transfers, rounds, and care planning.
• Coordinate follow-up appointments and transportation needs.
• Collaborate with residents, families, providers, and the interdisciplinary team.
• Support end‑of‑life care when needed.
• Promote teamwork, regulatory compliance, and survey readiness.

Compensation: $36 – $43, based on experience + applicable shift differentials

Sign On Bonus: $2,000

About the Role

We're looking for an LPN who combines strong clinical skills with compassion, communication, and teamwork. You'll provide direct nursing care while helping coordinate the day-to-day work of nursing assistants and other assigned team members.
