The Elms Rehabilitation and Skilled Nursing in Milford, NH, is hiring an LPN to provide direct nursing care and coordinate day-to-day nursing assistant activities. You’ll monitor residents, administer medications, and collaborate with families and the care team.

This role offers part-time shifts (7AM-3PM or 11PM-7AM) with compensation of $36-$42 per hour, plus sign-on bonuses for full-time and part-time positions. CPR cert and active NH LPN license required.

Formación

• Graduate of an accredited LPN/LVN program.
• Current unrestricted LPN license in the practicing state.
• Current CPR certification.
• Strong clinical judgment, communication, and leadership skills.
• Ability to organize priorities and remain composed in a changing healthcare environment.
• Prior long-term care experience preferred.

Responsabilidades

• Provide direct nursing care and supervise assigned nursing assistants and support staff.
• Monitor resident status and promptly communicate significant changes.
• Administer medications and monitor residents for adverse reactions.
• Perform wound care, treatments, and nursing procedures within scope of practice.
• Accurately carry out and document physician orders.
• Participate in admissions, discharges, transfers, rounds, and care planning.
• Coordinate follow-up appointments and transportation needs.
• Collaborate with residents, families, providers, and the interdisciplinary team.
• Support end-of-life care when needed.
• Promote teamwork, regulatory compliance, and survey readiness.

About The Role

We're looking for an LPN who combines strong clinical skills with compassion, communication, and teamwork. You'll provide direct nursing care while helping coordinate the day-to‑day work of nursing assistants and other assigned team members.
