We're looking for an LPN who combines strong clinical skills with compassion, communication, and teamwork. You'll provide direct nursing care while helping coordinate the day-to-day work of nursing assistants and other assigned team members.
From medications and treatments to monitoring resident status and collaborating with families and providers, you'll play an important role in ensuring residents receive safe, responsive care.
Provide direct nursing care and supervise assigned nursing assistants and support staff.
Monitor resident status and promptly communicate significant changes.
Administer medications and monitor residents for adverse reactions.
Perform wound care, treatments, and nursing procedures within scope of practice.
Accurately carry out and document physician orders.
Participate in admissions, discharges, transfers, rounds, and care planning.
Coordinate follow-up appointments and transportation needs.
Collaborate with residents, families, providers, and the interdisciplinary team.
Support end-of-life care when needed.
Promote teamwork, regulatory compliance, and survey readiness.
Graduate of an accredited LPN/LVN program.
Current unrestricted LPN license in the practicing state.
Current CPR certification.
Strong clinical judgment, communication, and leadership skills.
Ability to organize priorities and remain composed in a changing healthcare environment.
Prior long-term care experience preferred.
