Job summary

The Elms Rehabilitation and Skilled Nursing in Milford, NH is seeking a Licensed Nursing Assistant to provide hands-on daily living assistance and compassionate care for residents. You will collaborate with nurses to ensure dignity, comfort, and safety while maintaining accurate documentation and resident-centered care.

The role offers full-time and part-time shifts with opportunities for sign-on bonuses and career development within the 603 Healthcare network.

Qualifications

• Current LNA certification in good standing.
• CPR/BLS certification current.
• High school diploma or equivalent required.
• Minimum 1 year of LNA experience in long-term care preferred.

Responsibilities

• Assist residents with personal care and activities of daily living.
• Safely lift, move, position, and transport residents.
• Support residents during meals and provide feeding assistance as appropriate.
• Assist with toileting programs and related care.
• Monitor and report changes in resident condition, skin integrity, weight, and other care needs.
• Maintain accurate flow sheets and required documentation.
• Support admissions, discharges, and transfers.
• Communicate resident needs and changes promptly to nursing staff.
• Follow infection-control, safety, and emergency procedures.
• Promote resident dignity, comfort, independence, and quality of life.

Available Shifts

• Full-time 7AM - 3PM and 3PM - 11PM
• Full-time weekend baylor shift opportunities
• Part-time 7AM - 3PM

Compensation

$21 - $26 based on experience + applicable shift differentials and full-time mod comp options

Sign On Bonus

Full Time $5,000 | Part Time $2,500

Referral Bonus

Full Time $2,000 | Part Time $1,000

About The Role

As a Licensed Nursing Assistant (LNA), you'll be an important part of our residents' daily lives. You'll provide hands‑on care, assist with activities of daily living, communicate changes in resident condition, and help ensure every resident is treated with dignity, compassion, and respect. You'll work alongside nurses and other members of the care team to provide dependable, resident‑centered care.

What You’ll Do

What We’re Looking For

• Patience, professionalism, and a resident‑centered approach.
• At least one year of LNA experience in long‑term care preferred.
