Job summary

Greenbrier Senior Living is staffing The Rivers at Maryland Farms in Brentwood, TN for a Full-Time Night Shift LPN position (7:00 PM–7:00 AM). Two LPNs are scheduled each night, with one assigned to Assisted Living and one to Memory Care.

The Rivers at Maryland Farms is under new management by Greenbrier Senior Living. Offers a $2,500 sign-on bonus, aggressive benefits, and a supportive team-focused environment.

Qualifications

• Active Tennessee Licensed Practical Nurse (LPN) license
• Current CPR certification
• High school diploma or GED required
• One year of related nursing experience preferred
• Senior living, Assisted Living, or Memory Care experience is a plus

Responsibilities

• Provide direct resident care following individualized plans of care
• Administer medications per physician orders and policies
• Perform resident assessments as required by state regulations
• Monitor resident condition and communicate concerns promptly
• Document observations and care provided during shifts
• Maintain and update resident care plans
• Collaborate with the on-duty LPN and caregiving team
• Guide and support other associates as appropriate
• Contribute to a positive, team-focused environment

What You’ll Do

As an LPN at The Rivers at Maryland Farms, you’ll play an important role in providing exceptional care and creating a safe, comfortable environment for our residents.

• Provide direct resident care while following individualized plans of care.
• Administer medications in accordance with physician orders, state regulations, standards of practice, and community policies.
• Perform resident assessments as required by state regulations and community policies.
• Monitor residents for changes in condition and promptly communicate concerns to appropriate leadership.
• Document resident observations, clinical procedures, communications, and care provided during your shift.
• Maintain and update individualized resident care plans.
• Work collaboratively with the other LPN on duty and our caregiving team to ensure residents receive excellent care.
• Provide guidance and direction to other associates as appropriate.
• Help create a positive, supportive, and team-focused environment for residents and associates.

What We’re Looking For

• Active Licensed Practical Nurse (LPN) license in Tennessee
• Current CPR certification
• High school diploma or GED
• One year of related nursing experience preferred
• Senior living, Assisted Living, or Memory Care experience is a plus, but not required
• Strong communication and teamwork skills
• Ability to exercise sound clinical judgment and respond appropriately to resident needs
• Ability to meet the physical requirements of the position, including assisting residents with transfers and mobility using proper equipment
