As an LPN at The Rivers at Maryland Farms, you’ll play an important role in providing exceptional care and creating a safe, comfortable environment for our residents.
Provide direct resident care while following individualized plans of care.
Administer medications in accordance with physician orders, state regulations, standards of practice, and community policies.
Perform resident assessments as required by state regulations and community policies.
Monitor residents for changes in condition and promptly communicate concerns to appropriate leadership.
Document resident observations, clinical procedures, communications, and care provided during your shift.
Maintain and update individualized resident care plans.
Collaborate with our nursing, caregiving, and leadership teams to ensure residents receive excellent care.
Communicate effectively with residents, families, physicians, and other members of the care team.
Provide guidance and direction to other associates as appropriate.
Help create a positive, supportive, and team-focused environment for residents and associates.
