Job summary

The Rivers at Maryland Farms in Brentwood, TN is seeking an LPN for the day shift (7:00 AM–7:00 PM) under Greenbrier Senior Living's new management. You will provide direct resident care and administer medications to create a safe, comfortable living environment for residents.

Required: Tennessee LPN license, CPR certification, HS diploma or GED, and at least 1 year of nursing experience; senior living experience is a plus. $2,500 sign-on bonus and opportunities for growth.

Qualifications

• Active LPN license in Tennessee.
• Current CPR certification.
• High school diploma or GED.
• One year of related nursing experience preferred.
• Senior living, Assisted Living, or Memory Care experience is a plus, but not required.
• Strong communication and teamwork skills.
• Ability to exercise sound clinical judgment and respond appropriately to resident needs.
• Ability to meet the physical requirements of the position, including assisting residents with transfers and mobility.

Responsibilities

• Provide direct resident care following individualized plans of care.
• Administer medications per physician orders and regulations.
• Perform resident assessments as required by state regulations and policies.
• Monitor resident status and communicate changes to leadership.
• Document observations and care provided during shifts.
• Maintain and update care plans for residents.

What You’ll Do

As an LPN at The Rivers at Maryland Farms, you’ll play an important role in providing exceptional care and creating a safe, comfortable environment for our residents.

Provide direct resident care while following individualized plans of care.

Administer medications in accordance with physician orders, state regulations, standards of practice, and community policies.

Perform resident assessments as required by state regulations and community policies.

Monitor residents for changes in condition and promptly communicate concerns to appropriate leadership.

Document resident observations, clinical procedures, communications, and care provided during your shift.

Maintain and update individualized resident care plans.

Collaborate with our nursing, caregiving, and leadership teams to ensure residents receive excellent care.

Communicate effectively with residents, families, physicians, and other members of the care team.

Provide guidance and direction to other associates as appropriate.

Help create a positive, supportive, and team-focused environment for residents and associates.

What We’re Looking For
