Responsibilities

• Ensure quality, individualized health care management for individuals.
• Conduct routine site visits, check vitals, assess, counsel on medication administration, document progress.
• Serve as health/medical resource for staff and train staff in medical areas.
• Conduct audits and reviews of medications.
• Ensure compliance with state requirements and organizational policies regarding medical care and records.

Job Description

We are seeking a Licensed Practical Nurse (LPN) to help support the continuous enrichment and development of individuals with developmental disabilities in our adult residential facilities in Indianapolis, IN.

Schedule

• Full-time Monday-Friday with occasional travel required; flexible hours
• Participate in an on‑call rotation for triage of medical concerns for 1 week per month

Qualifications

What Makes You A Great Fit

• Current Licensed Practical Nurse (LPN) license in the State of Indiana required
• 1-2 years experience working with individuals with disabilities or other long-term care is preferred
• Must be skilled, flexible, self-motivated, dependable, and able to work cooperatively with staff, interdisciplinary team members, and outside agencies
• Strong verbal/written communication skills
• Ability to work independently and efficiently
• Organizational and computer skills
• Acceptable background check & driving record check
