Job summary

Avondale Health and Rehabilitation Center in Humboldt, TN is seeking compassionate Licensed Practical Nurses (LPNs) for the day shift. You will provide direct resident care, administer medications, and support the clinical team in a skilled nursing and rehabilitation facility.

Active Tennessee LPN license required with preference for long-term care experience. Weekend and holiday availability is expected. A $10,000 sign-on bonus is offered for full-time positions.

Qualifications

• Must hold an active Tennessee LPN license.
• Experience in long-term care, skilled nursing, or rehab preferred.
• Strong clinical and assessment skills.
• Excellent communication and teamwork abilities.
• Ability to prioritize and work in a fast-paced environment.
• Willingness to work weekends and holidays as needed.

Responsibilities

• Provide direct nursing care according to individualized care plans.
• Administer medications and treatments as ordered.
• Monitor resident condition including vital signs and changes in status.
• Complete accurate and timely documentation.
• Assist with admissions, transfers, and discharges.
• Work closely with RNs, CNAs, physicians, and therapy staff.
• Support residents with daily care needs when needed.
• Participate in care planning and family meetings.
• Follow infection control and safety procedures.
• Help train and support new nursing staff.

Avondale Health and Rehabilitation Center is seeking compassionate and dependable Licensed Practical Nurses (LPNs) to join our growing team in Humboldt, Tennessee. In this role, you will provide direct resident care, administer medications, and support our clinical team in delivering high-quality care in a skilled nursing and rehabilitation facility.

$10,000 Bonus for full time!

LPN Requirements

• Active Licensed Practical Nurse (LPN) license in Tennessee
