Dyersburg Health and Rehabilitation Center is seeking Licensed Practical Nurses (LPNs) to join our team in Dyersburg, TN. This full-time or PRN role offers a $6,000 sign-on bonus and opportunities to work in a skilled nursing and rehabilitation facility.

Qualifications

• Active Licensed Practical Nurse (LPN) license in Tennessee.
• Experience in long-term care, skilled nursing, or rehab preferred.
• Strong clinical and assessment skills.
• Excellent communication and teamwork abilities.
• Ability to prioritize and work in a fast-paced environment.
• Willingness to work weekends and holidays as needed.

Responsibilities

• Provide direct nursing care according to individualized care plans.
• Administer medications and treatments as ordered.
• Monitor resident condition including vital signs and changes in status.
• Complete accurate and timely documentation.
• Assist with admissions, transfers, and discharges.
• Work closely with RNs, CNAs, physicians, and therapy staff.
• Support residents with daily care needs when needed.
• Participate in care planning and family meetings.
• Follow infection control and safety procedures.
• Help train and support new nursing staff.

LPN Requirements
