Nashville Center For Rehabilitation and Healing is seeking a Licensed Practical Nurse (LPN) for full-time, rotating 12-hour shifts (7:00–7:00) to care for residents in our Skilled Unit and Long Term Care Unit.

• Licensed LPN in Tennessee.

Responsibilities

• Provide care for residents in the Skilled Unit (SNF) and Long Term Care Unit.

• Take and record vital signs.
• Provide resident care and treatments with monitoring of outcomes.
• Provide resident and family education.
• Communicate with physicians, NPs, families and other health care professionals.
• Supervise CNAs.
