Job summary

STAT Home Health is seeking a Full Time Licensed Practical Nurse (LPN) to provide nursing and supportive care in patients' homes in the New Orleans area. You will work under the supervision of an RN, coordinating with physicians and the home health team to ensure quality patient care.

Applicants must hold a Louisiana LPN license, have CPR certification, at least 1 year of LPN experience, and reliable transportation for home visits.

Qualifications

• Current Louisiana LPN license
• Minimum 1 year clinical experience as an LPN
• Current CPR Certification
• Compassionate and great time management skills
• Reliable transportation to conduct patient visits

Responsibilities

• Provide nursing and supportive care per the Interdisciplinary Plan of Care under RN supervision.
• Communicate with physicians, home health team, and other agencies to coordinate patient care.
• Document all services provided.
• Demonstrate knowledge of Medicare/Medicaid regulations for home health benefits.
• Report changes in patient condition to the RN
