Job summary

STAT Home Health in New Orleans, LA is hiring a full-time Licensed Practical Nurse (LPN) to provide compassionate care in patients' homes under the supervision of an RN. You will perform assessments, administer treatments, and document care plans to support recovery and comfort.

Qualifications

• Current Louisiana Licensed Practical Nurse (LPN) license required
• Minimum 1 year clinical experience as an LPN
• Current CPR Certification
• Compassionate and great time management skills
• Reliable transportation to conduct patient visits at their place of residence or facility

• Provide nursing and supportive care in accordance with the Interdisciplinary Plan of Care, under the direction of the RN
• Communicate with the attending physicians, other home health team members, and other agencies as needed to coordinate optimal care and use of community resources for the patient/family.
• Document all services provided.
• Demonstrate knowledge of Medicare/Medicaid regulations for home health benefits.
• Report changes in patient condition and needs to RN
