STAT Home Health in Houma, LA is hiring a full-time Licensed Practical Nurse (LPN) to deliver nursing and supportive care at patients’ homes under the supervision of the RN. You will coordinate with physicians and the home health team, document services, and ensure care aligns with Medicare/Medicaid rules while maintaining compassionate, timely patient visits.

Job Duties:

• Provide nursing and supportive care in accordance with the Interdisciplinary Plan of Care, under the direction of the RN
• Communicate with the attending physicians, other home health team members, and other agencies as needed to coordinate optimal care and use of community resources for the patient/family.
• Document all services provided.
• Demonstrate knowledge of Medicare/Medicaid regulations for home health benefits.
• Report changes in patient condition and needs to RN

Qualifications:

• Current Louisiana Licensed Practical Nurse (LPN) license required
• Minimum 1 year clinical experience as an LPN
• Current CPR Certification
• Compassionate and great time management skills
• Reliable transportation to conduct patient visits at their place of residence or facility
