Job summary

Green-Hills-Center-For-Rehabilitation-and-Healin is hiring a Licensed Practical Nurse (LPN) for 12-hour day and overnight shifts in our Skilled Unit and Long Term Care Unit. Responsibilities include medication administration, IVs, vital signs, and patient education, with ongoing communication to physicians, NPs, and families.

Qualifications

• Current Tennessee LPN license.
• Experience with medication administration and vital signs is required or preferred.
• Experience in SNF/long-term care is a plus.

Responsibilities

• Provide care for residents in SNF and Long Term Care unit.
• Administer medications, including IVs, and monitor outcomes.
• Take and record vital signs and report changes.
• Educate residents and families on care plans.
• Communicate with physicians, NPs, families and other health professionals.
• Supervise CNAs.
