Job summary

Holton Manor in Elkhorn, WI is seeking a licensed physical therapist to join our in-house therapy program. The role supports inpatient, outpatient and home health services with flexible scheduling and various shifts, including PRN options.

You will work with a team of Occupational, Physical and Speech therapists to provide high-quality care seven days a week. Competitive benefits and orientation are provided; compensation is dependent on experience.

Qualifications

• Graduate of an accredited physical therapy program.
• Licensed in Wisconsin.
• Must not be excluded or sanctioned from state or federal programs.
• Proficient administrative and communication within department and facility.
• Thorough knowledge of payer source documentation and payment requirements.
• Ability to use analytical skills to develop outcome driven solutions.

Responsibilities

• Uphold the professional, legal and ethical standards of practice for physical therapy as defined by the APTA and State Practice Act.
• Ensure delivery of quality services and outcomes to meet or exceed expectations of those utilizing or impacted by the department.
• Perform initial patient examination, evaluation, interpretation of referrals, and create the initial plan of care for each patient and revise as appropriate.
• Create and maintain a patient record for each patient treated.
• Provide on-site assessment and re-evaluation of each patient’s treatment according to required interval and adjust the plan as needed.
• Coordinate discharge plan with the PTA.
• Demonstrates knowledge and application of precautions and contraindications to PT treatment.
• Complete documentation according to facility policies including timeliness and medical necessity support.
• Has primary responsibility for PT care rendered by the PTA for all patients; delegate appropriate portions of the plan to the PTA.
• Must be accessible by telecommunication to the PTA while the PTA treats patients.
• Performs other duties as required and/or assigned.

This position will provide skilled therapy services for our growing Inpatient, Outpatient and Home Health Programs, and will be supported by our team of Occupational, Physical and Speech therapists. Key Responsibilities:

Must uphold the professional, legaland ethical standards of practice for his or her physical therapy as defined by the APTA and State Practice Act

Responsible for service and operational excellence of all assigned activities to ensure the delivery of quality services and/or outcomes requiredto meet or exceed the expectations of those utilizingor impactedby the department

Completes documentation according to Facility policies including timeliness and medical necessity support.

Has primary responsibility for physical therapy care renderedby the PTA for all patients/clients for whom he or she is the therapist of record. In doing so:

Delegates appropriate portionsof the treatment plan and program to the PTA consistent withthePTA’seducation, trainingand experience

Mustbe always accessible by telecommunication to the PTAwhile the PTA is treating patients/clients for whom he or she is the therapist of record

Performs other duties as requiredand/or assigned

Qualifications:

Graduate of an accredited education course in physical therapy.

Licensed in the state of Wisconsin.

Must meet all health, background check, and training requirements as required by Wisconsin law
