Holton Manor in Elkhorn, WI is seeking a licensed practical nurse (LPN) to provide safe, individualized nursing care to residents under supervision. You will administer medications, assess changes, document care in the EMR, and mentor CNAs in best practices.

The role offers flexible scheduling with day, evening, night shifts, on-site work, and opportunities for professional development through tuition reimbursement and ongoing training.

Qualifications

• Active LPN license in Wisconsin or eligible for WI licensure by reciprocity.
• CPR certification is required.
• Ability to meet physical and job-related requirements.
• Strong written and verbal English communication skills.
• Long-term care or skilled nursing experience preferred.

Responsabilités

• Provide safe, effective nursing care per care plans and orders.
• Administer medications and treatments accurately.
• Assess residents and report changes promptly to RN.
• Document care in the EMR system accurately.
• Monitor performance and mentor CNAs as needed.

Position – Licensed Practical Nurse (LPN)

Location: Holton Manor – Elkhorn, WI (On-site)

Status: Full-Time | Part-Time | PRN

Schedule: Days | PMs | NOCs | Weekends – True Flexible scheduling - you choose what fits your schedule

Job Summary

The LPN must be a clinically competent, licensed practical nurse who provides care to a group of patients/residents under general supervision/delegation of a registered nurse according to established policies, procedures, protocols and standards of practice and care. Effectively interacts with patients/residents, families, responsible party, and all health team members.

Compensation

• $28.00 – $32.83 per hour dependent on experience
• PLUS $2.00 per hour PM and $2.50 per hour NOC shift differentials

Key Responsibilities

• Provide safe, effective nursing care per care plans and physician orders
• Administer medications and treatments accurately
• Assess residents and report changes promptly
• Document care in the EMR
• Monitors performance and mentor CNAs
• Ensure infection control and safety compliance
• Completes all other duties as assigned.

Qualifications

Required:

• Active LPN license in state of Wisconsin or licensed in any U.S state and be eligible for Wisconsin licensure by reciprocity. (in good standing)
• CPR certified
• Ability to meet physical and job-related requirements
• Must be English literate and possess excellent written and verbal communication skills

Preferred:

• Long-term care or skilled nursing experience
• Clinical experience
