Job summary

Southeast Rehabilitation & Skilled Care Center in North Easton is seeking Licensed Practical Nurses (LPN) for full-time and part-time positions. You will provide direct nursing care to residents under RN supervision and supervise the daily activities performed by nursing assistants to ensure high‑quality care.

We offer openings on all shifts, including every other weekend rotation; compensation ranges from $38 to $43 per hour with shift differentials.

Qualifications

• Graduate of an approved LPN/LVN program.
• CPR certification required.
• Current, unencumbered LPN/LVN license in this state or in process.
• Minimum 1 year experience in hospital, long-term care facility, or related health care setting.
• Ability to read, write, speak and understand English clearly.

Responsibilities

• Provide direct nursing care to residents under supervision of the RN.
• Supervise day-to-day nursing activities performed by nursing assistants.
• Meet the nursing and rehabilitation needs of each resident.
• Maintain current resident care plans and follow physician's orders.
• Transcribe physician's orders promptly and accurately.
• Review patient records daily for accuracy and completeness.
• Document comprehensive nursing notes and visit residents daily.
• Maintain awareness of residents on the alert list and report findings.
• Document and report unusual findings; contact physician when necessary.
• Perform other duties as assigned by manager.

• Supervise the day-to-day nursing activities performed by the nursing assistant
• Meet the nursing and rehabilitation needs of each resident
• Maintain current resident care plans
• Follow physician's orders
• Transcribe physician's orders promptly and accurately
• Review patient records daily to assure accuracy and completeness
• Document comprehensive and complete nursing notes
• Visit each resident daily to evaluate resident needs
• Maintain accurate awareness of residents on alert list
• Document and report any unusual or significant findings; contact the physician when necessary
• Perform other duties as assigned by manager

Requirements:

• Must be a graduate of an approved LPN/LVN program
• Must be CPR certified
• Must possess a current, unencumbered license to practice as an LPN/LVN in this state, or be in the process of obtaining one
• Minimum, one (1) year experience in a hospital, long-term care facility, or other related health care facility (new graduates welcome!)
• Ability to read, write, speak, and understand the English language
• Ability to make independent decisions when circumstances warrant such actions
• Ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel, and the general public
